What it is
Strong beliefs that are not based in reality and remain fixed even when contradicted.
How it shows up
Believing something untrue (e.g., thinking someone is trying to harm you, or that familiar people are impostors), often with strong conviction.
Why it happens
Stroke can affect brain regions involved in perception, reasoning, and reality testing, particularly in the frontal and temporal lobes.
How common
Uncommon, but can occur in certain types of stroke or alongside other cognitive changes.
Possible management
Medical evaluation, psychological support, structured routines, and calm, reassuring communication.
