Confabulation
Tips from survivors: “It feels real in the moment — having someone calmly guide me back helps.”
What it is
Unintentional creation of false or inaccurate memories without the intent to deceive.
How it shows up
Filling in memory gaps with incorrect details, telling stories that feel real but didn’t happen, or mixing up timelines and events.
Why it happens
Stroke affects memory and self-monitoring areas of the brain, making it harder to distinguish between real and reconstructed memories.
How common
Less common, but can occur in strokes affecting frontal or memory-related brain regions.
Possible management
Gentle correction, structured routines, memory aids, and supportive communication from caregivers.
