What it is

A rare condition where a person believes someone close to them has been replaced by an identical impostor.

How it shows up

Recognizing a familiar face (like a spouse or family member) but feeling certain they are not really that person.

Why it happens

Stroke disrupts the connection between facial recognition and emotional familiarity, often involving the temporal and frontal lobes.

How common

Extremely rare, but documented in some neurological conditions including stroke.

Possible management

Neurological and psychological support, reassurance, structured routines, and gentle orientation strategies.
